HOME AND COMMUNITY BASED SERVICES

Home and Community Based Services, supported by the Department of Public Welfare, Office of Long Term Living, and the Medical Assistance Waiver, aim to empower eligible adults aged 18 and above with physical disabilities. The program is designed to enable these individuals to live independently, stay in their homes, and pursue and/or sustain employment. Waiver Coordination plays a crucial role in facilitating access to various supports and services within the Home and Community setting

Personal Care

For many adults facing physical disabilities or aging challenges, maintaining residence in their own homes becomes challenging without some level of assistance in personal care and light household tasks. These services play a vital role in preventing unnecessary institutionalization, especially when family or friends are unavailable or unable to provide the required support.

SavannahCare Waiver Coordination is free for financially eligible persons. For those over the eligibility limits, fees are based on a sliding scale. SavannahCare Waiver Coordination’s professional staff will be happy to assist individuals in identifying the funding sources which can help them take advantage of the many Home and Community Based Services available.
